Device not working in app

I was able to login in again after deleting app.....but now when I go to add device back....device shows up but all the buttons are red and I can't doing anything.....when I close app and restart the app...the device is gone and it's asking me to add a device.

Please send the Mac address of your device, we can deal with it. 

Where is the mac located on the device....I have the Storm LGR Extreme Blue......its in the crawl....but I have access to it.

App won't let me sign in anymore hoping to find Mac there
